SunKnowledge Helps Healthcare Providers Prepare for CMS Prior Authorization Mandate in 2027

The Centers for Medicare & Medicaid Services established new interoperability and electronic prior authorization requirements through the CMS Interoperability and Prior Authorization Final Rule (CMS-0057-F). Starting January 1, 2027, payers must implement and maintain a Prior Authorization Application Programming Interface (API) based on the Health Level Seven Fast Healthcare Interoperability Resources standard.

The mandate aims to simplify prior authorization by enabling providers to determine whether authorization is required, identify payer-specific documentation requirements, submit requests electronically, and receive responses through compatible electronic health record or health information technology systems. Impacted payers include Medicare Advantage organizations, state Medicaid and Children’s Health Insurance Program fee-for-service programs, Medicaid and CHIP managed care plans, and Qualified Health Plan issuers on federally facilitated exchanges. The rule also introduces an Electronic Prior Authorization measure for certain MIPS-eligible clinicians, eligible hospitals, and critical access hospitals participating in applicable Medicare Promoting Interoperability programs, requiring attestation that at least one qualifying prior authorization request was submitted electronically through a Prior Authorization API using certified EHR technology.

Some operational provisions begin before 2027. Starting in 2026, specified payers must issue expedited prior authorization decisions within 72 hours and standard decisions within seven calendar days, though the seven-day standard does not apply to Qualified Health Plan issuers on federally facilitated exchanges under this rule. Payers must also provide specific reasons for denials and publicly report selected prior authorization performance metrics.
